Duties and Responsibilities
Performs essential functions of a paramedic. Provides proactive care that is intended to help improve the overall health and wellness of program participants. Requires excellent rapport, understanding, and communication with the patients and their family members. Expands access to non-urgent health care through in-home patient visits and/or community-based services. Common services will include: assessment (vital signs, blood pressure, labs such as glucose levels, diagnostic cardiac monitoring, medication compliance and reconciliation, adherence to discharge instructions, etc.), treatment (wound care, medication administration, post-operative follow-up, etc.), prevention (immunizations, home safety check, health teaching, etc.), and referral (mental health, medical and social services, etc.). Thereby producing better health outcomes and reducing the number of ambulance transports, visits to the emergency department, and hospital readmission’s.

Conducts patient education, including diabetes prevention/treatment, hypertension, Congestive Heart Failure (CHF), Chronic Obstructive Pulmonary Disease (COPD), falls assessment, injury evaluation. Works in cooperation with other stakeholders/key medical providers in order to offer coordinated and integrated care (physician offices, home health agencies, hospice, long term care facilities, public health department, social services, etc.).

Serves as a trained “navigator” of community resources, connecting patients to agencies and services they may not be aware of (i.e. Home Health, Community Care of North Carolina, etc.). Coordinates appointments and follow-up with physicians and hospitals. Reduces barriers to care. Consistently demonstrates professional behavior and performs proactive interventions which support the Mission and Values of Johnston County EMS. Responds to patient complaints.Builds and advocates for the program. Correctly utilizes advanced protocols/procedures approved by the medical director. Provides outreach education to the general public as requested by administration. Documents visits in electronic medical record. Enters required information into client database in order collect and analyze statistics for evaluation plan.

Acts in the capacity of a Paramedic on an EMS unit as needed. Maintains all knowledge, skills, and competencies to perform this role. Ability to communicate effectively both orally and in writing. Ability to establish and maintain effective working relations with others. Inspect emergency vehicles used by EMS to ensure the safe operations and efficient use.

Maintains and ensures patient, personnel matters and department confidentiality.

Represents Johnston County EMS when assigned at meetings, dealing with public and educational presentations.

Knowledge, Skills and Abilities
Knowledgeable of Johnston County EMS system, geographical area of the County, subdivisions and other key locations. Maintains knowledge in current departmental policies, county policies, and N.C OEMS regulations. Knowledge of emergency health care procedures and ability to evaluate the patient’s conditions and properly render the appropriate treatment set forth in the patient care protocols, with or without physicians’ direction. Skilled in the operations of an emergency vehicle, using knowledge and skills in driving to avoid hazards or to keep from becoming a hazard. Ability to pass and maintain training in specialized courses pertinent to Emergency Medical Services. Thorough knowledge of laws, rules, regulations, techniques and practices involved in the operation of emergency medical services. Ability to plan and organize treatment in emergency and non-emergency situations. Ability to develop and maintain effective work relationships with coworkers, EMS volunteers, hospital and medical community professionals, the public, and other emergency agencies. Ability to exercise sound judgment and make effective patient decision and accurate paramedic medical diagnosis and treatment decisions.

PHYSICAL REQUIREMENTS
Must be able to physically perform the basic life operational functions of climbing, balancing, stooping, kneeling, crouching, crawling, reaching, standing, walking, pushing, pulling, lifting, fingering, feeling, talking, hearing, and repetitive motions. Must be able to perform heavy work exerting up to 100 pounds of force occasionally, and/or up to 50 pounds of force frequently, and/or up to 20 pounds of force constantly to move objects. Must be able to carry appropriate bags and equipment to handle an emergency situation. Must be able to lift at least 200 pounds with a partner, lift and carry appropriate emergency bags, and use proper carrying techniques when loading and unloading a patient. Must possess the visual acuity to operate an emergency medical ambulance or other vehicle and distinguish detail and differences when observing people, places and things on medical emergency calls.

Desired Education and Experience
Associate degree in a medically related field and experience as an EMT at the paramedic level: or an equivalent combination of education and experience. Johnston County EMS Preceptor.

SPECIAL REQUIREMENTS
Certification from the State of North Carolina as a Paramedic. Current certifications in CPR, ACLS, PALS/PEPP and BTLS-A/PHTLS Certified at paramedic level for at least one year or approved at paramedic level in Johnston County. Possession of a valid North Carolina driver’s license.
