If your credential has been amended through an administrative action, the timeframe and stipulations for you applying for reinstatement of the original credentialing level will be outlined in the action.  Please refer to the document for that information.

If your credential has been suspended or revoked, you may petition The NC EMS Disciplinary Committee (The Committee) and the NCOEMS Chief for reinstatement once the following occurs:

  2. You complete a federal fingerprint check.
  3. You submit a written request for a review to determine eligibility for credentialing.
  4. You submit a written explanation of the events of what occurred that led to the revocation or suspension and what factors have changed since the original action was issued.
  5. You submit evidence of rehabilitation, letters of reference and any other documents you wish The Committee and NCOEMS Chief to consider before a final decision is made on your credentialing eligibility.

  1. It’s been at least 24 months since the effective date of the final action OR at least 24 months from the last criminal charge or conviction (in NC or any other state).
